Lost Tools
Lost tools must be immediately reported to a supervisor. The supervisor and employee who lost the tool will attempt to find the tool before involving the FOD Manager or FOD Advisor. If the tool is not found, the part or vehicle that was being worked on needs to be quarantined until the tool has been found or it has been determined that the tool cannot be found and is not a FOD hazard. The FOD Manager is the only authorized person to make such a determination.

Regardless of the outcome, a root cause analysis is required to be performed and an action plan needs to be developed. An action plan can be as simple as a letter in the offending employees file, or as extreme as the FOD Manager determines necessary to ensure continual FOD safety.
All incidents must be documented and archived.
